Key Responsibilities:

• Deliver expert health education and coaching to members, enabling them to navigate the healthcare system with confidence
• Facilitate seamless transitions of care by coordinating services, referrals, and appointments
• Conduct thorough assessments to identify gaps in care and develop targeted interventions to address them
• Provide empathetic support and guidance to members, addressing their unique needs and concerns
• Cultivate strong relationships with members, providers, and internal stakeholders to drive positive health outcomes
• Utilize advanced clinical skills, critical thinking, and problem-solving to address complex member needs

Requirements:

• Current, unrestricted RN licensure
• 3+ years of clinical RN experience, with a strong foundation in clinical skills and critical thinking
• Proven expertise in telephonic communication, with excellent inbound and outbound skills
• Intermediate computer skills, including Microsoft Word, Outlook, and Internet navigation
• Ability to work in a fast-paced, dynamic environment, with a focus on delivering exceptional member experiences

Preferred Qualifications:

• Bachelor's degree in Nursing
• Coaching and/or decision support experience
• Managed care experience
• Conflict management skills
• Proven understanding of change management principles and the ability to motivate members towards positive health behaviors
