Remote Nurse Practitioner / Physician Assistant | WFH

Key Responsibilities • Deliver specialized Physiatry/PM&R services aimed at enhancing patient quality of life in SNFs. • Collaborate with a multidisciplinary team to manage various conditions including CVA, progressive neurological diseases, and post-operative orthopedic care. • Partner with physical, occupational, and speech therapists to monitor and improve patient outcomes. • Participate in facility meetings to provide leadership and bridge the gap between therapy and internal medicine. • Engage in ongoing learning and contribute to a community of like-minded professionals. Required Skills • A strong passion for post-acute care and working with the geriatric population. • Self-motivated with the ability to work independently and collaboratively. • Compassionate and empathetic approach to patient care. • Effective collaboration skills to work within a multidisciplinary team. Qualifications • Active Nurse Practitioner (NP), APRN, or Physician Assistant (PA) license in New Mexico. • Preferred experience in post-acute care settings. • Legal authorization to work in the U.S.

Compensation
