About the position The LTSS Service Coordinator-Auditor is responsible for auditing member individual service plans to ensure compliance with state laws and contractual obligations. This role focuses on assessing the appropriateness of service plans for members with long-term service and support needs, ensuring that both clinical and non-clinical needs are met efficiently. The position involves documenting findings, identifying issues, and providing training opportunities for staff. Responsibilities • Audit program needs assessments and member's long-term service and support needs to ensure established service plans are appropriate. , • Determine if members with potential clinical health care needs are coordinated with the clinical healthcare management and interdisciplinary team. , • Ensure non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities are met for cost-effective utilization of services. , • Audit short and long-term service and support goals for appropriateness of services and waiver programs. , • Document audit findings, identify issues, and inform management. , • Assess and identify training opportunities for staff. Requirements • BA/BS degree and a minimum of 2 years experience working with a social work agency including 2 years LTSS service coordinator experience. , • Equivalent combination of education and experience may be considered. Nice-to-haves • BA/BS degree in a health care related field preferred. , • Specific education and experience may be required based on state law and contract requirements. , • Willingness to travel to worksite and other locations as necessary.
