As a Case Management Analyst, you will be a critical liaison between members, providers, and community resources, ensuring that each member receives coordinated, person‑centered care. Key Responsibilities

Member Assessment & Eligibility

  • Utilize arenaflex’s proprietary case‑management tools and data analytics platforms to conduct comprehensive assessments of member needs, eligibility, and benefit utilization.
  • Identify high‑risk factors and social determinants that may affect health outcomes, and prioritize interventions accordingly.
  • Develop individualized care plans that align with member goals, clinical guidelines, and arenaflex’s quality standards.

Care Coordination & Navigation

  • Facilitate timely scheduling of medical appointments, specialty referrals, and ancillary services.
  • Guide members through the complexities of Medicare and Medicaid enrollment, ensuring they receive all entitled benefits.
  • Collaborate with providers, social service agencies, and community partners to address non‑clinical needs such as housing, nutrition, and transportation.

Quality Assurance & Documentation

  • Maintain accurate, up‑to‑date case notes in compliance with regulatory requirements and arenaflex’s internal policies.
  • Monitor care‑plan progress, track key performance indicators, and adjust strategies to meet defined outcomes.
  • Participate in multidisciplinary case reviews, presenting member cases to clinical leadership and advocating for necessary resources.

Member Education & Advocacy

  • Provide clear, empathetic education on health conditions, medication management, and lifestyle modifications.
  • Empower members to become active participants in their own care by offering decision‑making support and self‑advocacy tools.
  • Utilize motivational interviewing techniques to encourage behavior change and adherence to treatment plans.

Continuous Improvement

  • Identify systemic barriers to care and propose process enhancements to senior leadership.
  • Stay current on federal health‑policy updates, Medicaid waivers, and best practices in dual‑eligible care.
  • Contribute to the development of training materials and knowledge‑sharing sessions for the broader arenaflex team.

Essential Qualifications

  • Minimum 3 years of experience in a health‑related field, preferably in case management, care coordination, or health‑services navigation.
  • Demonstrated ability to assess complex health and social needs using data‑driven tools.
  • Strong analytical skills with a keen eye for detail and the ability to interpret eligibility criteria.
  • Excellent written and verbal communication skills, with a focus on empathy and clarity.
  • Proficiency in Microsoft Office Suite and familiarity with electronic health record (EHR) or case‑management platforms.
  • High school diploma or equivalent; a Bachelor’s degree in health administration, public health, or a related field is preferred.

Preferred Qualifications & Certifications

  • Professional certifications such as CRC (Certified Rehabilitation Counselor), CDMS (Certified Diabetes Management Specialist), CRRN (Certified Rehabilitation Registered Nurse), COHN (Certified Occupational Health Nurse), or CCM (Certified Case Manager).
  • Direct experience with Medicare Advantage, Medicaid, or dual‑eligible programs.
  • Background in geriatric care, chronic disease management, or disability services.
  • Knowledge of health‑risk assessments, screenings, and care planning methodologies.
  • Bilingual proficiency (English/Spanish or English/Creole) to serve diverse member populations.
