Role Overview St. Jude Behavioral Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. This full-time remote position is integral to providing support and guidance within our Hospice Center, ensuring that our patients and their families receive the highest standard of care during challenging times.

Key Responsibilities

  • Provide comprehensive psychosocial assessments and develop tailored treatment plans for patients and their families.
  • Facilitate individual and group therapy sessions, utilizing evidence-based practices to enhance patient well-being.
  • Collaborate with interdisciplinary teams to coordinate care and resources effectively.
  • Assist families in navigating the emotional and logistical challenges associated with hospice care.
  • Maintain accurate and timely documentation in compliance with organizational policies and regulatory standards.
  • Engage in continuous professional development and stay updated with best practices in social work within hospice care.

Required Qualifications

  • Master's degree in Social Work from an accredited institution.
  • Current and valid LCSW licensure in New Hampshire.
  • Minimum of 2 years of clinical experience, preferably in hospice or palliative care settings.
  • Strong interpersonal skills and a genuine passion for helping others.
  • Ability to work independently in a remote environment while managing a caseload effectively.
