Scheduled Hours 40 Position Summary Provides individual attention to each patient and needed support by navigating patients through the complexities of the screening, diagnostic, treatment, and maintenance care continuums.

Job Description

Primary Duties & Responsibilities: Serves as a resource to assist patients and family navigate care continuum, barriers to care, and identifies resources/interventions related to social determinants of health (SDOH). Tracks interventions and outcomes. Completes and processes all necessary paperwork and documentation to enroll patients in appropriate programs. Facilitates communication and education with heath care staff and providers. Performs other duties as assigned. Working Conditions Job Location/Working Conditions Normal office environment.
