Role Responsibilities

• Conducting medication reviews to enhance patient safety and treatment effectiveness.
• Processing acute medication requests and making informed clinical decisions.
• Reviewing hospital discharge summaries to ensure accurate medication reconciliation.
• Assisting GP practices in achieving QOF and IIF targets as needed.
• Contributing to the management of long-term conditions within the multidisciplinary team.
• Delivering medicines optimisation and structured medication reviews.

Requirements

• Must be an Independent Prescriber
• GPhC registration
• Strong communication skills
• Self motivated
• Experience working in GP practices
